You may edit them once you've downloaded the file. Please save the document as the student's name, attach and send via email. You may use the email button above to send these documents to me upon enrollment. They may also be attached and sent in reply from your enrollment verification email.
This Notice of Termination should be sent to your local school board alternative education liaison if your students were previously registered as homeschoolers with your county. Please do not send this to me.
